The best possible surgical outcomes occur when you and your family participate in your treatment. Dr. Slam and her staff will make every effort to answer your questions and address your concerns before and after surgery.

You can help us by providing important information about yourself and your health. Before your first appointment, please complete the following forms and bring them with you.

Dr. Slam does her best to stay on time and to respect her patient’s waiting time, but as with everything in medicine, emergencies can arise.  She tries to give each patient the time they need to fully evaluate their condition on each office visit.  If the office staff notices she is running a little behind, they will do their best to alert you of this at check-in.  Rarely will she have to cancel office for a surgical emergency in the hospital, but if she does, you will be rescheduled as quickly as possible.

WHAT TO BRING

  • Medical records, breast imaging on film or CD, and other pertinent medical information that can help Dr. Slam understand your current and past medical conditions
  • A list of any medications you currently take and list of medication allergies
  • A list of your questions and concerns
  • Your insurance card (on every visit)
  • Your office visit co-payment

ARRIVAL TIME

For new patients, please arrive at least 30 minutes before your scheduled appointment time for computer check-in.  Established patients may arrive 15 minutes prior to your scheduled appointment, as long as you have been in the office within the past 6 months. You’ll receive a phone call one business day before your appointment as a reminder.

CANCELLING AN APPOINTMENT

If you must cancel an appointment, kindly give us 24 hours notice so that we may accommodate another patient with urgent needs. As all patients receive a computerized phone call 24 hours prior to the office visit, a $25.00 no-show charge may be incurred for failure to cancel a scheduled office visit appointment. A $150.00 charge may be incurred for failure to cancel a scheduled surgical procedure.  Patients may be released from care if there is a pattern of no-showing to office appointments.
